Anthony would like to personally thank each and every person who participated and supported his Doughnut Fundraiser. At this time, due to Anthony’s recent surgery and recovery all certificates purchased will be mailed to each individual who purchased. Please be sure to provide your correct information on the form below in order to have your certificates mailed to the correct address and also in a timely manner. All Certificates will begin the mailing process on January 18th, 2022! To receive updates concerning the delivery or mailing of your certificates please provide your correct e-mail address.

Due to His recent surgery and recovery, Anthony will not be meeting with or allowing any pickup of Doughnut Certificates! No Exceptions! Your prayers, support, and understanding are appreciated during this time!

If you have a purchased a CD & or shirt, Anthony & His team will be mailing your certificates to you once Physical Copies are delivered in his possession, to prevent double mailing and to cut down on cost concerning mailing and shipping. Your patience and cooperation are greatly appreciated during this time.
